A familiar is an equippable NPC that can accompany you in majority of the fights. Each one of them has different ability that are automatically used during their turn in combat.

If the familiar's HP reaches 0, they will immediately flee for the duration of the battle.

The Shadow's Vanity perk from Librum Umbra Malum Ovilla will prevent you from using all familiars except the shadow bird and shadow rat.

Leveling

Every time you win a fight, the familiar equipped gets 1 XP (even if the familiar is knocked out the battle early). Every 3 XP, the familiar gets +1 Max HP. Every 20 XP, the familiar gets +1 to all stats. Familiar Chow can also be used for +3 Max HP and +1 to all stats.

toilet gator	Fish for him in the toilet at Radio Shack.	Max HP: 9 Muscle: 5 Mysticality: 1 Moxie: 1	3 Physical Armor; Deals its Muscle in physical damage to a random enemy.
shadow bird	At Shadow Taint Level 4, grab the shadow egg in the shadow rift at Barbara Yargara's Hut Basement.	Max HP: 13 Muscle: 1 Mysticality: 3 Moxie: 2	Increases one of the player's stats by its Mysticality and does its Moxie in Spooky damage to all enemies.	This will replace the bird familiar you have. Choose wisely. Also missable if you decide to go the violent way against Barbara Yargara.
nutcracker	Random drop in Government Ballet Theater and Government Valley Wander Events.	Max HP: 11 Muscle: 4 Mysticality: 3 Moxie: 2	Deals its Muscle in physical damage to a random enemy and reduces that enemy's stats by its Mysticality.
maniacal doll	Random drop in Government Amphitheater and Government Valley Wander Events.	Max HP: 5 Muscle: 1 Mysticality: 3 Moxie: 5	Deals its Muscle in physical damage to a random enemy and applies its Moxie in Bleeding.
